Responsibilities

   • Completes and initial assessment of patient and family to determine wound care needs.
   • Provides a complete assessment and history of current and previous illnesses that are relevant to the wound care plan.
   • Initiates the wound plan of care which establishes goals based on diagnosis. Incorporates therapeutic, preventative and rehabilitative actions.
   • Demonstrates knowledge and expertise in clinical decision-making for the wound therapy patient population
   • Shall be accessible by telecommunications during operating hours when patients are receiving services.
   • Receives wound care referrals, reviews available patient information related to case, including disciplines required to determine home care needs.
   Coordinates wound services for managed care patients, assists with the establishment of the plan of care in collaboration with the physicians and the UM Case Managers to ensure a seamless transition to discharge.
   • Collaborates closely with the practitioners and case managers as to the recommended plan of care and regarding any change in condition.
   • Consults with the Clinical Director to address and resolve any concerns regarding the delivery of appropriate, timely patient care and education.
   • Educates and mentors other staff in current wound care treatments.

Required Qualifications

   • Current California Masters of Physical Therapy and Board Certified Wound Care Specialist Certification.
   • Two years of recent experience as a Board Certified Wound Care Specialist.
   • Basic computer including Excel and Word.
   • BLS required within 60 days of hire.
   • Possess critical thinking skills, ability to work independently and as a member of an interdisciplinary team.
   • Good communication skills. Excellent customer service skills.
   • Current California driver's license and proof of auto insurance.
   • Prefer someone bilingaul in Spanish
